Below is the Community Application for FASST Services in English and Spanish. This is a one page information sheet that is completed with the family and returned to one of the FASST programs by either fax or mail.
Download our FASST Area Map Below to help determine which FASST provider agency serves your area. Use the zip codes that you reside in if your child is between the ages of birth and 5 years. If your child is in elementary school, grade K - grade 5, then use the zip code that the school is in (you can also refer to the list of schools under each agency on the map for quick reference.
